HLS Opt-Out Form
All students will participate in Healthy Life Skills (HLS) instruction unless the parent/guardian chooses to withdraw (opt out) their child from all or portions of the program. If the parent/guardian chooses to withdraw their child, the Opt-Out form(s) must be completed before instruction begins. Parents/guardians will be notified prior to the start of instruction.
