Homebound Services:
Academic instruction provided to students who are confined at home or in a health care facility for periods that would prevent normal school attendance based upon medical certification of need by a licensed physician or licensed clinical psychologist.
Homebound instruction is not intended to replace the regular school curriculum, and is therefore, not a guarantee that the student will progress in the academic program.
HCPS Protocol for Homebound Services:
- If you have questions or concerns about your child's eligibility to receive homebound services, please start communication with your child's school counselor or administrator.
- Completed homebound paperwork is completed by parent and physician and sent to the Homebound Coordinator, Chloe Jerlinski, at HCPS Central Office.
- Homebound Coordinator collaborates with the assigned medical provider to approve/decline request along with start/end dates.
- Once a teacher is assigned homebound instruction may begin within 5 days.
- School counselor is typically the main school contact between school, homebound staff, and family.
- Homebound may be approved for up to 9 weeks. If an extension is necessary, the physician will need to complete the medical extension form.
- Based on student need, a transition meeting may be required within the early stages of homebound instruction to determine student's treatment goals, anticipated school support, and transition plan to return to a regular school setting.
BOTH the parent form and the physician form must be completed and returned to the child's school to apply for homebound services
